NON AVAILABILITY DEATH CERTIFICATE – GHMC
GHMC, registers and maintains the Deaths taken place. Non availability of Death Certificate is required for the following reasons.
• Death was not officially recorded. • Death records have been lost at Dept in natural calamities.
Documents Needed :
1. Application Form*
2. Ration Card/Any Other Residence Proof
3. Other Documentary evidences to prove the place
4. In case of Medical Legal cases a) FIR b) PM Report
c) Form_2 given the concerned police station
5. Notarized Affidavit (Non-Judicial Stamp Paper)
6. Any Other Supporting Documents
