DEATH CERTIFICATE - CDMA
The registration of deaths is one of the major functions of Public Health Section of the ULB. Generally the deaths, which take place in the hospital are recorded and reported to the municipalities. Death certificate service facilitates the citizen in registration of Death through Mee Seva.

Service Charge: 35/- +10/- (Panchayath),+50/- (Municipality up to 4 copies,100/- for 5 to 10 copies)
Statutory Charge : 50/-
Total Charge: (Rs) 85/- +Panchayat Rs10(Municipality up to 4 copies,100/- for 5 to 10 copies)
